Little Smiles Dental has adopted an internal grievance procedure providing for prompt and equitable resolution of complaints alleging discrimination under Section 1557 of the Affordable Care Act.
Filing a Complaint
Any person who believes they have been subjected to discrimination may file a grievance within 60 days of the alleged incident.
Grievances should be submitted to the Practice Administrator using the contact information listed above.
Investigation Process
- Complaints will be acknowledged within 5 business days
- An investigation will be conducted promptly
- A written response will be provided within 30 days
Resolution
If additional time is required, the complainant will be notified in writing.
Appeals
If the complainant is not satisfied with the outcome, they may appeal within 15 days of receiving the decision. A final determination will be issued within 30 days of the appeal.

You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights.
